This is one of the most common patterns I see in clinic, particularly in women over 40.

Many describe it as:

• “I wake up flat, but by 3pm I feel swollen”
• “By evening I look 5 months pregnant”
• “My clothes feel tighter as the day goes on”

It can be frustrating, uncomfortable, and often confusing, especially if you’ve been told everything is “normal.”

The good news is that this pattern is usually quite common.


Why Bloating Builds Through the Day

Bloating that worsens as the day goes on is rarely random.

It usually reflects a combination of:

1. Fermentation in the Gut

As you eat throughout the day, certain foods ferment in the digestive tract.

If the gut is not breaking food down efficiently, gas builds up gradually — leading to visible distension.


2. Slower Gut Motility

After 40, digestive movement often slows.

This means food sits longer in the digestive tract, increasing the chance of bloating and pressure.


3. Hormonal Changes

During perimenopause and menopause, shifts in oestrogen and progesterone can affect:

• Fluid balance
• Gut sensitivity
• Digestive rhythm

This is why many women notice bloating suddenly worsening in their 40s and 50s.


4. Digestive Capacity

Lower stomach acid and enzyme production can reduce how well food is broken down.

This leads to more fermentation later in the day.


5. Eating Patterns

Common habits that contribute include:

• Eating quickly
• Eating while stressed
• Large raw meals (especially salads)

These may seem healthy — but can worsen bloating in some people.


Why It Often Gets Labelled as IBS

Many people with this pattern are told they have IBS.

While IBS can be a useful label, it doesn’t always explain:

• Why symptoms worsen at a certain time of day
• Why they appeared later in life
• What specifically is driving them

In many cases, there are clear underlying contributors that can be addressed.


Simple First Steps That May Help

While individual assessment is important, a few simple changes can reduce afternoon bloating:

• Allow time between meals (avoid constant grazing)
• Eat slowly and in a relaxed state
• Avoid very large raw meals
• Support regular bowel movements
• Stay well hydrated earlier in the day

These are not quick fixes. However, they are part of a structured approach that supports better digestion.
